This vintage (80's) sewing pattern is Butterick 6381, to create a blouse with a triangle-shaped cutout at the front collar.  This is a style I have seen just lately on the racks!   This pattern gives you sleeve and design options though, to truly make it your own.  Find it at Pokey Pokey Needlecraft Shop:
